Corrective Care Program Information

WHAT IT INCLUDES:

  • All chiropractic adjustments on your program
  • Necessary X-rays
  • Re-examination and re-evaluations
  • Telephone consultations and/or office consultations
  • Lifestyle classes and consultations

In order to help as many people as possible, adjusting times are for adjusting ONLY. If you have a concern and would like further consultation, or if the Doctor finds you require additional information, that will be scheduled by phone or in person at a special time.

WHAT IT DOES NOT INCLUDE:

  • Supplies, supports, cervical traction devices, nutrition, ice packs, supplements, etc.
  • Care for work-related accidents or auto accident injuries, trauma, re-aggravation, or delayed healing time

PAYMENT POLICIES:

  • Payments can be made in full or we will extend a payment plan for our patients in order to be able to afford care.
  • Payments must be paid by recurring credit or debit card in order to qualify for care plan pricing. If cash payment an extra 10% fee will be added to monthly payment.

DISCONTINUATION:

  • In the event you choose to discontinue care for any reason, or we regretfully find it necessary to discharge you from care, any outstanding fees become immediately due and payable.
  • Retail fees will be calculated without discounts and pro-rated on a per visit basis. You do not simply owe your monthly fee. Retail fees are calculated at the per visit rate as laid out on your care plan.

INSURANCE AND YOUR CORRECTIVE CARE PLAN:

  • Only the functional care amount will be billed to the insurance company
  • Most insurance companies do not cover all charges or the full amount of the charges

VIDEO AND AUDIO:

  • We are recording video and audio at the same time 24 hours a day on all premises for yours and our teams safety.
  • We use recording of videos and audio from classes on our websites and social, YouTube and other means for patient education and follow ups. You understand that by attending classes and being in our office your face and voice might appear on these videos.

FAMILY/CHILDREN POLICY:

  • We ask that all children be checked within two weeks of a parent/spouse starting care

VISIT POLICY:

  • We ask that patients attend all classes in the first ninety (90) days of starting care
  • The visit frequency must be kept by the patient according to the schedule
  • On the occasion that you do have to miss an appointment, please call. It significantly reduces the amount of work on the front desk staff and allows us to reschedule your next visit and keep you getting/staying well
  • Missed appointments must be made up within 24 hours
  • Patient and/or Doctor vacations and seminars will be made up by increasing visits before and/or after the scheduling change
